2. Click on the picture to the left (you know, the one that looks like an order form) to download it.
3. If it doesn't open automatically, open the file you just downloaded (the one called "nickelAndDimeOrder.pdf").
4. If you click in each field in the form, you can type in the appropriate values.
5. Please fill in all fields for each item you want to order: quantity, format, item description and price (if you order more than one of an item, be sure to multiply the price by how many you're ordering).
6. When you have filled in all the information for the items you want, look at the shipping rates and fill in the postage amount accordingly.
7. Add together all the items you've ordered and postage required, and fill in the total price.
8. You might want to put your name, address, city, state, and country. If you don't, we get your money and you get nothing.
9. Be sure to fill out the alternate selections in case we run out of stock on an item between the time you've ordered it and the time we receive it.
10. Print it out, send it to us in a crusty old envelope with your cash or money order and we'll get your order out to you ASAP.
